About the Book: As nurses in clinical, academic, and community settings grapple with the real challenges facing the nursing profession outlined in the Future of Nursing 2020-2030, learn how one innovative nursing team successfully implemented strategies to transform care delivery to meet the Triple Aim Goals: improving patient experience of care, improving health of populations, and reducing per capita healthcare costs. This journey included partnering with primary care providers and specialists, home care and skilled nursing facilities and community agencies to integrate care delivery and improve quality outcomes. Partnerships with local nursing schools resulted in clinical rotations and curriculum development to expand student's perspective and preparation for value-based care and population health.

Especially as healthcare costs are outpacing the economy, healthcare systems are shifting from fee-for-service to value-based payment to deliver high-quality care while reducing costs. This shift presents nurses with the opportunity to take the lead in transforming care delivery. INSPIREd Healthcare follows author Billie Lynn Allard and her team of nurses as they successfully implement an accountable community of health in pursuit of the Triple Aim. The INSPIRE Model they follow provides an evidence-based blueprint for other healthcare systems hoping to solve the complicated problems surrounding care transitions and health promotion and serves as a model for future nursing curriculum development.
